Care Coordination: Interdisciplinary Rounding module

This module is intended to be a guide for operational leaders, managers, and point of care staff for self-assessment and implementation of a process for interdisciplinary rounds. It provides an assessment tool for measuring progress, informaiton to help teams adapt and adopt best practices, and templates to support various aspects of the process.
