Join the movement to enable Canadians to age with dignity at home
Most Canadians want to live and age in their homes for as long as possible – yet our system isn’t set up to support this.
Without formal supports to allow older adults age in community, they face lengthy waitlists for long-term care and often need to rely on emergency services for non-emergency medical care in the meantime.
This isn’t sustainable for Saskatchewan – but together, we can take steps to change that.

In partnership with Healthcare Excellence Canada, we’re looking for community organizations to join the movement to make aging in place a reality for older adults with complex medical needs who are impacted by structural and social determinants of health, such as what they look like, where they live or how much money they have.
This new pan-Canadian collaborative, called Enabling Aging in Place, is designed to help organizations implement their own programs using promising practices that have successfully helped older adults stay at home safely across Canada.
What are the benefits of joining?
- Seed funding up to $15,000 in Phase 1 and $25,000 in Phase 2
- Virtual and in-person networking, coaching, and learning events
- Evaluation and measurement support
- Resources and capacity building supports
- Opportunity to guide and inform the development of new resources to aid aging in place programs and serve the specific needs of your community
Who should apply?
This opportunity is for Saskatchewan organizations who are developing or expanding an aging in place program that supports older adults with complex medical needs who are impacted by structural and social determinants of health, and therefore are most likely to be admitted to long-term care without formal support at home. This includes, but isn’t limited to:
- Primary care providers and clinics
- Community-based organizations
- Long-term care and continuing care organizations
- Social or private housing
- Acute care facilities
- Municipalities
- Regional health authorities
- Indigenous governments and organizations

What can you expect from the program?
Leveraging an Asset-Based Community Development (ABCD) approach, you’ll develop and implement an aging in place plan that will help improve safety, health, and quality of life for older adults and delay entry to long-term care, reducing unnecessary emergency department visits and demands on care providers while optimizing health and social care resources.
Phase 1
November 2023 – April 2024
- Grow relationships and partnerships
- Develop an implementation and evaluation plan
- Build capacity for asset-based community development
Phase 2
June 2024 – September 2025
- Implement or expand promising practices to support aging in place
- Measure impact
- Build a plan to sustain results
How to apply
Fill out the electronic application form and submit it to EAIP-FVCS@hec-esc.ca. Applications for Phase 1 will be accepted until November 15, 2023.
Successful applicants of Phase 1 will be notified in December 2023. There will be a separate application process for Phase 2.
